Nominee’s Package

The Nominee must include all of the following required components:

  • A description of the nominee’s accomplishments explaining why the nominee is deserving of an award, (two pages maximum).
  • Support materials such as certificates, newspaper articles, photographs, etc., (6 pages maximum) or 3 minutes maximum on CD or DVD for media/performing arts.
  • 1 “head shot” photograph, high resolution, jpg on CD
  • 3 – 4 pictures of the nominee at work, and/or the nominee’s artwork, on CD
  • Self addressed return envelope for the return of material
